Acitretin is an oral retinoid used for severe psoriasis and other keratinisation disorders when topical treatments or other therapies are not sufficient. It is a vitamin A derivative and influences the growth and differentiation of skin cells. Treatment is powerful and should be used only under the supervision of a dermatologist.
It is taken by mouth, usually once daily with food, at the dose set by the doctor. The effect develops gradually over weeks or months. Blood tests for liver function and blood fats are needed, because acitretin can increase triglycerides and affect the liver. Alcohol may need to be avoided, because it can prolong pregnancy-related risks in women.
Side effects are common and include dry lips, mouth, nose and eyes, peeling, hair loss, fragile nails, muscle or joint pain, sun sensitivity, nosebleeds and increased blood lipids. Night-vision problems, severe headache or signs of liver injury may occur. Blood donation is usually prohibited during and after treatment for a defined period.
Acitretin can cause serious birth defects and is strictly contraindicated in pregnancy. Women who can become pregnant must follow the pregnancy prevention programme, tests and contraception for the recommended time, including after stopping. Contact a doctor urgently for possible pregnancy, severe headache, blurred vision, jaundice, intense abdominal pain or marked depression.
