Hydroxocobalamin is a form of vitamin B12 used to prevent or treat deficiency, including associated anaemia and nerve damage. It is particularly important when intestinal vitamin absorption is impaired. Separately, a much higher-dose intravenous preparation is used as an antidote for cyanide poisoning. These two uses are not interchangeable.
For deficiency, it is usually injected into a muscle, more frequently at first and then as maintenance treatment. Some conditions require lifelong replacement. The schedule depends on the cause, symptoms and blood tests. Poisoning treatment is an emergency managed exclusively by healthcare professionals using the appropriate preparation.
Injection pain, nausea, diarrhoea, rash or allergic reactions can occur. Potassium may fall when significant anaemia is first treated, requiring monitoring. Antidote doses can temporarily turn the skin and urine red, and the medicine's colour can interfere with some laboratory results.
Tell the doctor about cobalt allergy because vitamin B12 contains cobalt, and mention pregnancy, breastfeeding and other treatments. Seek emergency help for breathing difficulty, facial swelling or fainting. Report palpitations or worsening numbness and walking problems. Do not stop maintenance just because tiredness has improved: the cause of deficiency may still be present.
