Icodextrin is used in peritoneal dialysis solutions for people with kidney failure. Its sustained osmotic effect draws water into the dialysis fluid, helping remove excess fluid during the longest exchange. It is part of a dialysis plan prescribed by a kidney specialist, not a treatment that cures the kidneys.
The solution goes through a catheter into the peritoneal cavity, usually for one exchange daily. It must not be injected into a vein. Volume and dwell time are individualised. Home use requires training and aseptic technique. Do not use damaged bags or cloudy solution, and inspect the fluid when it drains out.
Possible problems include skin rash, allergic reactions, abdominal pain and changes in fluid or salt balance. Abdominal pain, fever or cloudy drained fluid may indicate peritonitis: contact the dialysis team immediately. Breathing difficulty or rapidly developing swelling also needs urgent assessment.
Icodextrin can cause falsely high glucose readings on certain meters, even for up to two weeks after stopping. This can lead to dangerous insulin administration. Use only compatible glucose meters and test strips confirmed by the care team, and tell every healthcare professional, including emergency staff. Do not adjust insulin on the basis of a suspicious reading.
