South African geranium root extract is a herbal medicine traditionally used to relieve symptoms of the common cold, such as blocked nose, mild cough, sore throat and general discomfort. It is obtained from the root of Pelargonium sidoides or Pelargonium reniforme. It is not an antibiotic and does not replace treatment for severe bacterial infections.

It is taken by mouth as drops, solution, tablets or syrup, according to the leaflet. In Europe it is regarded as a traditional herbal medicinal product for cold symptoms, based on long-standing use. If symptoms last longer than one week, worsen, or high fever occurs, medical advice is needed rather than continuing self-treatment.

Side effects may include nausea, abdominal pain, diarrhoea, heartburn, mild nosebleeds, bleeding gums, rash or allergic reactions. Liver problems have been reported very rarely, although the relationship may be difficult to prove. Caution is advised in patients with liver disease, anticoagulant treatment or a history of plant allergies.

Stop taking it and seek medical help if yellowing of the skin or eyes, dark urine, unusual tiredness, severe abdominal pain, breathing difficulty or facial swelling occurs. During pregnancy, breastfeeding and childhood, use only according to the permitted age and the advice in the leaflet or from a healthcare professional.
