Hexoprenaline relaxes the muscles of the womb by stimulating beta-2 receptors. In obstetrics it is used to delay premature labour briefly in selected pregnancies between weeks 22 and 37. The time gained allows measures such as treatment to support the baby's lung maturation or transfer to a specialist centre.
It is given intravenously in hospital under obstetric supervision, for no longer than 48 hours for this indication. Monitoring includes pulse, blood pressure, ECG, blood glucose, potassium and the baby's condition. It is not a home maintenance treatment. Oral forms should not be used to suppress premature labour.
Possible effects include palpitations, a fast pulse, tremor, sweating, headache, nausea and low blood pressure. Blood glucose may rise and potassium may fall. Serious reactions include abnormal heart rhythms, reduced blood supply to the heart and fluid collecting in the lungs. Careful patient selection and close monitoring are therefore essential.
Tell staff immediately about chest pain, breathlessness, fainting or an irregular heartbeat. Inform the doctor about heart disease, diabetes, an overactive thyroid and all medicines used. Treatment is unsuitable when prolonging pregnancy would endanger the mother or baby. The obstetric team must decide whether it is appropriate.
