Isosorbide dinitrate is a medication from the class of organic nitrates, used for the prevention and treatment of angina pectoris (chest pain caused by reduced blood flow to the heart). It is also used in some cases for congestive heart failure, as it helps reduce the workload on the heart by dilating blood vessels.

It works by relaxing and dilating blood vessels (both arteries and veins), improving blood flow to the heart and reducing the oxygen demand of the heart muscle. This mechanism helps prevent angina episodes and alleviate symptoms.

Isosorbide dinitrate can be administered as oral tablets, sublingual tablets (placed under the tongue for rapid absorption), or as a spray. Sublingual tablets are used for the rapid relief of acute angina episodes, while oral forms are used for long-term prevention.

Common side effects include headaches (especially at the start of treatment), dizziness, low blood pressure (hypotension), nausea, and facial flushing. These symptoms usually decrease as the body adjusts to the medication. Rarely, allergic reactions or severe hypotension may occur.

Patients should avoid alcohol during treatment, as it can enhance the hypotensive effects of the medication. Additionally, isosorbide dinitrate should not be used in combination with medications for erectile dysfunction (such as sildenafil), as the combination can cause a dangerous drop in blood pressure.
